Insurance
If you have BlueCross/BlueShield PPO:
I am an in-networks and I will submit claims for you. It is a good idea to call your insurance company and check on the exact benefits that are covered by asking the questions below. Please bring your insurance card to your first session.
If you have another insurance provider:
Services may be covered in full or in part by your health insurance through "out of network provider benefits." Please check your coverage carefully by asking the following questions:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment
Payment is due at the time of service. Cash, credit card, and personal check accepted for payment.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
